"−X% Target Defense" lowers the defense of the monster you attack by that percentage, which raises your chance to hit it. It changes your chance to hit, not your damage.
It matters most against high-defense monsters, where a low Attack Rating misses often. Serpent Lord, Razortine and Grim's Burning Dead give −50%.
