The game ships a starter profile for each class in its Loot Filter screen. Here is each one rule by rule, in the screen's own words, with every category opened up to the items it holds — and a builder that writes your own profile as a code to import on that screen.
Twelve rules, all switched off as shipped: the profile is a starting point to switch rules on in game. The broad HIDE ALL rule comes first and the Show rules after it.
The eight starters are the same file except rule 10, Hide Other Class Items, which lists the other seven classes' item categories — the Warlock's own is the one left out.
Every category is the game's item UI categories table: a branch (Equipment: Armor, Weapons, Accessories; Items: Misc) covers everything under it.

Start from a starter or an empty profile, tick the boxes the in-game screen has, and take the profile code into the game. The code is checked as you go; the buttons stay off until it is valid.
Copy the code and paste it into the import box of the game's Loot Filter screen, where it becomes a new profile. The .fltr file holds the same code.
The game's screen shows the selected profile's code in its export box: paste it here (or a .fltr file's text) to edit that profile.
A profile is JSON: a name and an ordered list of rules. Each rule has a name, an on / off switch, Show or Hide, the Ethereal / Socketed box, and the ticked Rarity, Quality, Equipment and Items boxes; empty lists are left out. The eight starter files parse and write back byte for byte.
The Equipment and Items trees are the item UI categories table: an item type names its category, an item can override it, and ticking a branch (Armor, Weapons, Accessories, Misc) covers everything under it.
Sources: the eight data/hd/global/excel/starter-*.fltr files, itemuicategories.txt, itemtypes.txt, misc.txt and the ui and item-name string tables of build 3.3.93847.
