Pick a base and where its item level comes from — typed in, a monster kill, a crafted item or a gamble — and read its affix level, the most sockets it can have at that level, what the cube's socket recipe and Larzuk give, and the magic affixes that can roll.

Tal Rune + Amn Rune + Perfect Ruby + a normal, unsocketed shield: at item level 85 1 (16.7%), 2 (16.7%), 3 (16.7%), 4 (50%).
Each affix with its level and weight (the table's frequency) and its share of its kind. Two affixes of the same group never roll on one item.
An item's level is the level of the monster that drops it: champions 2 above their type, uniques, their minions and super-uniques 3 (a super-unique at its home area's level + 3 on Nightmare and Hell). In a Terror Zone the monster is first raised to your level + 2 within the zone's bounds; a herald adds its tier's treasure-class boost. The treasure class upgrades at that level.
Output item level = the recipe's level + character level × plvl % + input item level × ilvl %; crafted recipes are 50 % and 50 %.
The affix level is the item level (at least the base level, at most 99) less half the base level, or twice the item level less 99 near the top; a base with a magic level adds it instead.
A base holds at most its own gemsockets, and at most its item type's MaxSockets1 for item levels up to 25, MaxSockets2 up to 40 and MaxSockets3 above. A Phase Blade: 6, but 3 below item level 26 and 4 below 41.
The four socket recipes (body armor, weapon, helm, shield; normal, unsocketed items) roll 1 to 6 sockets at even odds; the roll is then held to the base's maximum at its item level, so the maximum takes every roll at or above it.
Larzuk gives a low-quality, normal or superior item the maximum for its item level; a magic item one or two at random, a rare, set or unique item one. The quest reward is set in the game's code, not in a table.
A base fits a runeword when its item type, followed up the Equiv chain, is one of the word's types and none of its excluded ones, and it can hold the word's socket count. The lowest item level is where that count first fits.
A prefix or suffix can roll when its level is at most the affix level, its maximum level (if any) at least it, it is spawnable with a frequency, and the base's type chain holds one of its types and none of its excluded ones. Its frequency is its weight within its kind.
