Allows you to throw two different throwing weapons at the same time.
Double Throw is a Barbarian Combat Skill (level 12, requires Double Swing; it unlocks Frenzy). It needs a throwing weapon in each hand (skills.txt itypea1 and itypeb1 are both "thro") and throws both at a single target in one sequence. Its level buys damage, +16% at level 1 and +168% at level 20, and attack rating, +210% at level 20, for 1 mana a weapon.

The damage bonus is skills.txt Param1 (16) plus Param2 (8) a level: +16% at level 1, +48% at level 5, +168% at level 20, +248% at level 30; it reads lvl, so +skills count. Each hard point in Double Swing adds a further 8% (Param8; that term reads blvl). The Basin's page marks the whole damage line as a Resurrected addition: its older table has none.
The attack-rating bonus is ToHit 20 plus LevToHit 10 a level, the steepest in the tree: +20% at level 1, +210% at level 20, +310% at level 30. Mana is 1 at every level; the Basin's page says it is charged once per weapon in the sequence, so the tooltip prints 2, and the skill can still be started with 1 mana.
Each throw is a ranged weapon attack that rolls Chance to Hit against the target and deals that weapon's physical damage with Double Throw's bonus added to your other Enhanced Damage. The Basin says the attack-rating bonus is applied, unlike the bow skills of the original game. Both weapons go to the one selected target; if no target is selected, nothing is picked for you. Throwing Mastery applies to the thrown weapons (its attack rating, damage, critical-hit chance, pierce and the chance not to consume quantity), the melee masteries do not.
The Basin's page says the character screen adds the Double Swing synergy and Throwing Mastery to your other +% damage one after another instead of summing them, so the displayed damage can be up to three times what you actually deal. Read the table above, not the screen.
The Basin adds that throwing axes and throwing knives are medium-range weapons, hitting targets within about 14 2/3 yards, while javelins reach anything on screen.
That line reads blvl; Double Throw's own damage and attack-rating lines read lvl, so +skills from items raise both. Double Throw feeds nothing; it is the prerequisite of Frenzy.
A player's hit on another player is cut to 17% of its value (difficultylevels.txt PlayerDamagePercentVSPlayer) before anything else, see Player versus Player. Each thrown weapon is cut the same way; nothing else about Double Throw changes.
